Prescription Service - Private Prescription

Step 4: What happens next?

You have chosen to send us your prescription without waiting for a quote, and are happy with your own cost calculations.

We require a completed Patient Registration Form to be sent with your prescription.

Our Patient Registration form requests all the information we need to process your prescription. This information has to be included when you post your prescription to us.

You can download and print a blank form if you have Acrobat Reader or Microsoft Word on your computer.
If not, you can download Adobe Reader software for free.
